The instructions below are strictly for minor applicants

Application Requirements
Identification
Scanned copy of Childs Nigerian passport data page or birth certificate and US passport data page
(Provide Childs foreign passport ID or Passport data page as well)
If your passport is missing, submit a police report and a soft copy of the missing passport data page. If the data page is unavailable, provide the passport number..
Photograph
One (1) passport photograph is required. The photograph must meet the US standard passport requirements: 2x2 inches, with a white background.
Parental Identification
A copy of both parents' IDs are needed. Both parents should also provide the data pages of their Nigerian and US passports.
if both parents don't have a US passport, the permanent residency card (green card) is also accepted.
If one parent does not have a US passport, they may submit a valid ID or a foreign passport and an additional ID.
Single Parent Documentation
If only one parent is applying, a legal custody document for the child must be provided.
Travel Information
A flight ticket or flight itinerary is required as part of the application.
Consent Letter
A consent letter from both parents, or from a single parent in relevant cases, must be included. This letter must be signed and notarized.
