  2. Passport Payment slip

  3. Appointment slip

  4. Copy of current (expired/expiring) Passport data page

  5. One (1) Passport Photograph

  6. Expired/expiring Passport booklet 

  7. Marriage certificate or divorce documents as applicable

  8. Husband’s means of Identity​

  9. National Identification Number (NIN)
