2. Completed online application form 

3. Passport Payment slip

4. Appointment slip

5. One (1) Passport Photograph

6. Birth Certificate (Original may be required for sighting)

7. Parent’s passport data page

8. Consent letter signed by parent and duly notarized (single parent should provide Sole custody document from a court) 

9. National Identification Number (NIN)
