The Requirements below are for applications submitted for change of data information (name/dob etc)

Read Carefully

  1. Completed online application form 

  2. A formal application letter requesting for correction to be made and stating reason for change

  3. Passport Payment slip

  4. Appointment slip

  5. One (1) Passport Photograph

  6. Copy of current (expired/expiring) Passport data page

  7. Expired/expiring Passport booklet

  8. Court order for change of names and newspaper publication

  9. National Identity Number
