  2. Completed online application form 

  3. Passport Payment slip

  4. Appointment slip

  5. Evidence of Nigerian citizenship (state or local government identification certificate or evidence of birth by parent with Nigerian citizenship by birth)

  6. One (1) Passport Photograph

  7. Birth Certificate (Original required for sighting)

  8. Parents’ Nigerian passport data page 

  9. National Identification Number (NIN)
